U.S. PASSPORT INSTRUCTION SHEET – REQUIREMENTS - new fees 2-1-08
- BIRTH CERTIFICATE certified by Vital Statistics Office or Original Naturalization Certificate or Expired U.S. Passport (call for eligibility) Document will be retained and forwarded to the U.S. Passport Agency
- IDENTIFICATION – NJMVC Driver’s License and certain types of Employee ID (call for further information about acceptable ID)
- PHOTOGRAPHS – TWO 2” X 2” identical professional white background only
- SOCIAL SECURITY NUMBER is required information on the application
- FEES: age 16 and over:
$75.00 CHECK OR MONEY ORDER ONLY: U.S. DEPARTMENT OF STATE
$25.00 PROCESSING FEE- PAID SEPARATELY to the ATL. COUNTY CLERK
FEES CONTINUED: age 15 and under:
$60.00 Check or M.O. ONLY: U.S. DEPARTMENT OF STATE
$25.00 PROCESSING FEE paid separately to the ATL. COUNTY CLERK
EXPEDITED FEE IS AN ADDITIONAL $60.00. THERE MAY ALSO BE ADDITIONAL EXPRESS MAIL RETURN POSTAGE FEES IF REQUIRED.
IMPORTANT NOTICE FOR MINOR APPLICANTS !!
Minors age 15 years and under- require BOTH Mother and Father to sign in person (unless there is a sole custodial, consent order/affidavit) AND “Long Form” Birth Certificate which indicates BOTH PARENT'S names on the birth certificate.
ALL MINORS (regardless of age) and ALL INDIVIDUALS – MUST APPEAR IN PERSON. The only exception to this would be adults who are eligible to use the DS-82 called an “Application For Passport By Mail” (read eligibility requirements carefully)
